The RN Clinical Team Leader is responsible for assessing, planning, implementing, monitoring, and evaluating client care in a home health and hospice setting. They coordinate resources to ensure quality, compliance, and cost-effective outcomes, adhering to regulations and organizational policies.

Qualifications

• Virginia RN license (BSN preferred)
• Current CPR certification
• Minimum of one year in acute care and home health, with prior supervisory experience preferred
