Registered Nurse, RN, Hospital to Home Program

Responsibilities

• Conduct patient assessments and develop care plans
• Provide advanced skilled nursing treatments and patient education
• Coordinate care with physicians and interdisciplinary teams
• Monitor patient progress and adjust care plans as needed
• Document visits efficiently using AI-assisted tools
• Common treatments may include:
  • TPN administration
  • Chest tube management
  • Lab draws
  • IV therapy and maintenance
  • Wound vacs and wound care
  • Post-surgical recovery support
  • Respiratory care
  • Patient and caregiver education

Qualifications

• Active, unencumbered Florida RN license
• Valid driver’s license, reliable transportation, and auto insurance
• One year of med/surg, ICU, ER, or post-acute care required
• Successful completion of AHCA Level 2 background screening

Pay

Competitive compensation $70,000‑$74,888 based on experience and 36‑hour work week

Schedule

Set weekly schedule: choose between Monday‑Tuesday‑Wednesday 7 am‑7 pm or Tuesday‑Thursday‑Friday 7 am‑7 pm; three 12‑hour days; no weekend rotations or on‑call.
