Activa Home Health is seeking experienced Registered Nurses for our innovative Hospital at Home Program. This opportunity is designed to attract top clinical talent for full-time and per diem roles across multiple service areas, including Vero Beach, Port St. Lucie, and Stuart. We are looking for highly skilled RNs to deliver acute-level care in the home setting, supporting patients who would otherwise require hospitalization. This role is ideal for nurses who thrive in dynamic environments, deliver high-acuity care in the home, complete timely and accurate clinical documentation in Kinnser (WellSky), and are passionate about advancing patient-centered care models.

Responsibilities

• Provide high-quality, acute-level nursing care in a home setting as part of the Hospital at Home program
• Collaborate with interdisciplinary teams to monitor patient status and respond to changes in condition with strong clinical judgment
• Deliver advanced clinical interventions while ensuring patient safety and satisfaction
• Accurately document care in electronic medical records (EMR)
• Adhere to program workflows, response expectations, and professional standards

Requirements

• Active Florida Registered Nurse (RN) license
• Recent hospital-based experience required (Acute Care, ICU, ER, or Step-Down)
• Experience in high-acuity home health or complex medical care strongly preferred
• Proven ability to collaborate within interdisciplinary clinical teams
• Strong critical thinking, assessment, and communication skills
• Comfort with mobile technology and electronic documentation systems
• Proof of up-to-date vaccinations
