About the role
Extended Care Team (ECT) Nurse providing hospice care in Effingham and surrounding areas.
Responsibilities
- Provide comprehensive nursing care within your scope of practice, including assessments, treatments, and medication administration.
- Conduct patient and family assessments to contribute to individualized care plans.
- Respond to patient needs by completing various visit types and communicating changes to physicians.
- Coordinate with community health partners and ensure necessary equipment and services are available.
- Maintain accurate clinical documentation, monitor pain and symptoms, and support seamless care transitions.
- Continuously enhance clinical skills through ongoing education and training.
Requirements
- Active RN license in the state of practice.
- Graduate of an accredited Nursing program.
- Preferred experience in hospice, home health, or related fields.
- Strong assessment, clinical judgment, and documentation skills.
- Adaptability to varied patient environments and assignments.
- Valid driver’s license and willingness to travel locally.
