Responsibilities
- Provide end-of-life care to hospice patients and their families
- Develop and manage comprehensive patient care plans
- Collaborate with interdisciplinary teams to ensure holistic care
- Document patient care activities accurately and timely
- Attend regular team meetings and training sessions
Requirements
- Bachelor’s degree in Nursing or related field
- Current RN license in good standing
- Experience in hospice care or palliative care preferred
- Excellent communication and interpersonal skills
- Ability to work flexible hours including evenings and weekends
Qualifications

- Proficient in electronic health record systems
