Registered Nurse (RN) - Hospice, Outpatient, PRN, Days

Provide compassionate hospice and palliative care to terminally ill patients of all ages, assessing physical, emotional, psychosocial, and spiritual needs and developing individualized care plans in collaboration with patients and families.

Responsibilities

• Assess patient physical, emotional, psychosocial, and spiritual needs and develop individualized care plans in collaboration with patients and families.
• Perform thorough assessments, manage symptoms, and document findings accurately and timely.
• Monitor patient progress, update care plans, and refer to interdisciplinary team members as needed.
• Provide patient and family education, support end‑of‑life planning, and ensure patient wishes are respected.
• Oversee CNA interventions, ensure safe care delivery, and participate in team meetings and care planning.
• Support bereavement follow‑up and participate in quality improvement initiatives.
• Educate the public on end‑of‑life issues and mentor new staff or volunteers.
• Communicate effectively with team members, physicians, and families to coordinate care.

Qualifications and Skills

• Associate degree in Nursing; BSN preferred.
• At least one year of RN experience; hospice or home care experience preferred.
• Completion of a nurse residency may substitute for experience.
• Current RN license recognized in the state of practice, valid driver’s license, and auto insurance.
• Critical thinking, problem‑solving, and strong communication skills.
• Knowledge of grief/loss processes and hospice standards of care.
• Ability to collaborate in patient care, adhere to policies, and support families through end‑of‑life stages.
