Registered Nurse (RN) - Hospice, Outpatient, PRN, Days

Seeking a Registered Nurse to provide compassionate hospice and palliative care to terminally ill patients of all ages. Key Responsibilities Include Assessing patient physical, emotional, psychosocial, and spiritual needs, and developing individualized care plans in collaboration with patients and families.Performing thorough assessments, managing symptoms, and documenting findings accurately and timely.Monitoring patient progress, updating care plans, and referring to interdisciplinary team members as needed.Providing patient and family education, supporting end-of-life planning, and ensuring patient wishes are respected.Overseeing CNA interventions, ensuring safe care delivery, and participating in team meetings and care planning.Supporting bereavement follow-up and participating in quality improvement initiatives.Educating the public on end-of-life issues and mentoring new staff or volunteers.Communicating effectively with team members, physicians, and families to coordinate care. Qualifications And Skills Associate degree in Nursing; BSN preferred.At least one year of RN experience; hospice or home care experience preferred. Completion of a nurse residency may substitute.Current RN license recognized in the state of practice, valid driver’s license, and auto insurance.Critical thinking, problem-solving, and strong communication skills.Knowledge of grief/loss processes and hospice standards of care. This role involves collaborative patient care, adherence to policies, and supporting families through end-of-life stages, contributing to the organization’s mission to inspire health and serve with compassion.
