Registered Nurse (RN) - Hospice Outpatient - PRN, Day

About the Role

Provides hospice and palliative care utilizing the nursing process to terminally ill patients of all ages, supporting their families. Ensures compliance with organizational policies, regulatory standards, and scope of practice.

Responsibilities

• Assess patient physical, psychosocial, developmental, and environmental needs; contribute to interdisciplinary care plans.
• Perform age-appropriate physical assessments and symptom management, including pain control.
• Document findings accurately and communicate timely with physicians.
• Develop and update individualized care plans involving patients and families.
• Provide education and support to patients and families, including end-of-life planning.
• Coordinate care with team members, oversee CNA interventions, and ensure quality documentation.
• Participate in bereavement follow-up, team meetings, and community education on end-of-life issues.
• Supervise and evaluate staff and volunteers, fostering effective team collaboration.
• Maintain clinical competency and adhere to policies and procedures.
• Perform other duties as assigned.

Requirements

• Associate degree in Nursing; BSN preferred.
• Minimum one year RN experience; hospice or home care experience preferred.
• Current RN license in good standing.
• Valid driver’s license and auto insurance.
• Critical thinking, problem-solving, and excellent communication skills.
• Knowledge of grief/loss processes and hospice standards of care.
• Must participate in team-based care, demonstrate cultural competence, and support organizational mission.

Schedule

Primarily during daytime hours.

This is a non-management position reporting to a supervisor or manager.
