About the role

Provides specialized nursing care as a Registered Nurse (RN) in hospice settings, supporting terminally ill patients and their families in home or skilled nursing facilities.

Responsibilities

  • Assess patient needs, preferences, and barriers to care, including emotional, spiritual, and cultural factors.
  • Develop, implement, and document personalized care plans in collaboration with the healthcare team, patients, and families.
  • Promote respectful and empathetic interactions with diverse populations, ensuring patient-centered care.
  • Evaluate patient conditions, communicate changes with families and team members, and update care plans accordingly.
  • Ensure evidence-based practice and quality outcomes through ongoing assessment and critical thinking.
  • Coordinate care as a case manager when assigned, overseeing all aspects of patient care.

Qualifications & Skills

  • Licensed RN with a valid driver's license.
  • Associate's degree in Nursing required; Bachelor's degree preferred.
  • Less than 2 years of experience required; 2-5 years preferred for advanced roles.
  • Prior hospice or admissions experience is advantageous.

Schedule

  • Full-time position supporting hospice patients in the Greater St. Louis area, with a set schedule and weekend options.
