Registered Nurse (RN) - Hospice

Responsibilities

• Conduct initial nursing evaluations to determine hospice eligibility within 48 hours of referral.
• Assess and re-assess patients' physical, psychosocial, and environmental needs at least every 15 days.
• Develop, coordinate, and document the patient's Plan of Care in collaboration with interdisciplinary team members.
• Provide patient and family education on self-care techniques and monitor their response to care.
• Maintain accurate clinical notes on patient condition and interventions.
• Collaborate regularly with patients, families, physicians, and the interdisciplinary team to ensure quality care.
• Evaluate personal and professional performance to improve productivity and quality of services.

Qualifications & Skills

• Current, unrestricted RN licensure in the practice state.
• CPR certification or ability to obtain within 90 days.
• Valid driver's license, reliable vehicle, or access to transportation.
• Ability to work effectively in diverse home environments.
• Preferred: 1+ years of clinical experience.

Compensation & Benefits

• Salary ranges from $71,640 to $107,460 annually, based on experience and location.
