About the Role

Provide hospice care in home and facility settings, delivering skilled nursing focused on symptom management, patient comfort, and quality of life for seriously ill or terminal patients.

Responsibilities

• Conduct comprehensive patient assessments
• Develop and update individualized care plans
• Educate patients and families on care and symptom management
• Collaborate with an interdisciplinary hospice team
• Provide dignified end-of-life care and support
