Registered Nurse (RN) / Hospice

About the Role

Provide compassionate, comprehensive end-of-life care in patients' homes within Effingham and surrounding areas.

Responsibilities

• Conduct comprehensive and ongoing patient assessments to identify evolving needs.
• Develop and adjust personalized care plans collaboratively with physicians and care teams.
• Administer medications and treatments according to physician orders.
• Maintain accurate, timely, and meaningful clinical documentation.
• Serve as the communication link between families, physicians, and community health partners.
• Educate patients and caregivers to promote confidence and support.
• Participate in on-call rotations as per team policy.

Requirements

• Active RN license in the state of employment.
• Graduate of an accredited nursing program.

Skills

• Preferred experience in hospice, home health, or related fields.
• Strong clinical judgment and ability to work independently.
• Excellent communication skills with care teams and families.
• Compassionate approach to end-of-life care.
