About the role

The Home Health Registered Nurse (RN) at Presbyterian SeniorCare Network Home Health conducts initial nursing assessments using the OASIS tool, focusing on physical and psychosocial aspects of patients, families, and caregivers. They lead the home health team in developing and individualizing care plans, collaborate with physicians, and review comprehensive care plans with the team.

Responsibilities

• Conducts initial nursing assessments using the OASIS tool
• Develops and individualizes care plans with the home health team
• Collaborates with physicians to review and update care plans
• Provides leadership and guidance to the home health team
• Identifies and addresses the needs of patients and their families

Requirements

• Possesses an RN license in good standing from the State of PA
• Holds and maintains an active driver’s license and reliable transportation insured in accordance with Pennsylvania requirements
• Maintains CPR certification as per policy
• Works well with people and demonstrates enthusiasm and motivation
• Self-directed, prioritizes tasks, and manages time effectively
• Willing to contribute to a compassionate, resident-focused culture

Pay

Starting Rate: Pay is by Productivity Point, based on experience.
