Registered Nurse (RN) - Home Health Hospice

About the role

Provide compassionate end-of-life care in a home health hospice setting. Deliver holistic, patient-centered care addressing medical, emotional, spiritual, and psychosocial needs.

Responsibilities

• Collaborate with a multidisciplinary team to evaluate patient status, develop and implement care plans, and coordinate services.
• Monitor and document patient conditions, recommend additional services, and facilitate community resource referrals.
• Build meaningful, trusting relationships with patients and families during challenging times.
• Participate actively in interdisciplinary planning to ensure appropriate care and support.
• Provide end-of-life care, consult with the team, manage patient health conditions, and foster supportive, empathetic relationships.

Qualifications & Skills

• Current RN license in Maine or a compact state.
• Strong critical thinking, decisive judgment, and minimal supervision.
• Technology proficiency, with experience in electronic medical records preferred.
• Valid driver’s license with acceptable driving record.
• New graduates encouraged to apply.
