Registered Nurse (RN) - Home Health Hospice

About the role

Provide compassionate end-of-life care in a home health hospice setting. Deliver holistic, patient-centered care addressing medical, emotional, and spiritual needs.

Responsibilities

• Collaborate with a multidisciplinary team to evaluate patient status, plan, implement, and assess care plans.
• Provide direct treatment, monitor patient conditions, and make recommendations for additional services and community resources.
• Build meaningful connections with patients and families, offering comfort and support during challenging times.
• Participate in interdisciplinary planning to ensure appropriate care and resource utilization.
• Provide end-of-life care, collaborate with teams, evaluate patient needs, and ensure quality and compassionate support for patients and families.

Qualifications

• Current RN license in Maine or from a compact state.
• Strong critical thinking, decisiveness, and ability to work independently.
• Proficiency with technology and electronic medical records preferred.
• New graduates encouraged to apply.
• Valid driver's license with acceptable driving record.
