Registered Nurse (RN) - Home Health

Seeking a compassionate Registered Nurse (RN) for a home health role supporting patient-centered care through in-home visits, telehealth consultations, and administrative tasks. Responsibilities include: conducting assessments, reviewing medications, performing clinical interventions (medication administration, wound care, point-of-care testing, phlebotomy, Foley catheter care, IV fluids), documenting patient information, providing education, supporting care transitions, and participating in interdisciplinary team meetings. Qualifications: active RN license, minimum 3 years of direct patient care experience, CPR certification, proficiency with telehealth platforms, electronic health records, and Microsoft Office. Prior home care, hospice, palliative, or geriatrics experience is preferred. Must possess a valid driver’s license and auto insurance. Skills required: excellent communication, organizational, and time management skills; ability to work independently; cultural competence; physical ability to lift up to 20 lbs and assist with patient transfers.
