Location: Avera Dialysis-Sioux Falls. Primarily days with night, weekend, and holiday on-call involvement.
Position Highlights

About the Role
Provides major portions of self-care and home dialysis training and monitoring for dialysis patients and/or their caregivers. Be part of a multidisciplinary team built with compassion and the goal of moving health forward for you and our patients.
Responsibilities
- Assesses patient pre-therapy to determine if patient meets selection criteria.
- Identifies patient/family learning needs; develops, implements, and evaluates an educational plan for patients.
- Provides patient education related to the dialysis process, procedures, care, and management of dialysis accesses.
- Monitors and evaluates patients, on a minimum of a monthly visit, to problem-solve and mentor patients and families to safely deliver home therapy.
- Problem-solves and facilitates appropriate resources for patients on home therapy.
- Coordinates care of patients in collaboration with Medical Director/Nephrologist, patients, families, other nursing staff, and ancillary departments.
- Maintains accurate documentation and computer skills.
- Complies with established departmental policies and procedures, scope of care, performance improvement activities, safety, environmental, and infection control standards.
- Documents in the patient record according to departmental policy/procedure.
- Promotes and maintains documentation and an environment compliant with state, federal, and regulatory agencies, including CMS/ESRD-Renal Network, and Quality Assurance and Process Improvement efforts.
- Facilitates patient supply re-order.
Requirements
- Ability to work the specified hours.
- Visual acuity adequate to perform position duties.
- Ability to communicate effectively, hear, understand, and distinguish speech and other sounds.
Qualifications
- Required:
- Registered Nurse (RN) – active license in the state of practice (upon hire).
- 1-3 years of nursing experience.
- Preferred:
- Certified Hemodialysis Nurse (CHN) – Board of Nephrology Examiners Nursing and Technology (BONENT) (upon hire).
- Certified Peritoneal Dialysis Nurse (CPDN) – Board of Nephrology Examiners Nursing and Technology (BONENT) (upon hire).
- Certified Dialysis Nurse (CDN) – Nephrology Nursing Certification Commission (NNCC) (upon hire).
- Specialty Certification – The American Society of Histocompatibility & Immunogenetics (ASHI) (upon hire).
- Dialysis experience or critical care experience.

Pay
The pay range for this position is $32.50 - $48.50 per hour, dependent upon experience.
