- Regular Work Shift: Primarily days with night, weekend and holiday on-call involvement
About the role
Provides major portions of self-care and home dialysis training and monitoring for dialysis patients and/or their caregivers.
Responsibilities
- Affirms patient pre-therapy to determine if patient meets selection criteria.
- Identifies patient/family learning needs; Develops, implements and evaluates an educational plan for patients; Provides patient education related to but not limited to the dialysis process and procedures related that process, including care and management of dialysis accesses.
- Makes monthly visits to monitor and evaluate patients, problem solving/mentoring patients and families to safely deliver home therapy.
- Facilitates problem-solving and appropriate resources for patients on home therapy.
- Collaborates with Medical Director/Nephrologist, patients, families, other nursing staff and ancillary departments.
- Maintains accurate documentation and computer skills.
- Complies with established departmental policies and procedures, scope of care, performance improvement activities, safety, environmental and infection control standards.
- Documents in the patient record according to departmental policy/procedure.
- Promotes and maintains documentation and an environment that is in compliance with state, federal and regulatory agencies, including CMS/ESRD-Renal Network, and Quality Assurance and Process Improvement efforts.
- Facilitates patient supply re-order.
Requirements
- The individual must be able to work the hours specified.
- To perform this job successfully, an individual must be able to perform each essential job function satisfactorily including having visual acuity adequate to perform position duties and the ability to communicate effectively with others, hear, understand and distinguish speech and other sounds.
Qualifications
- Registered Nurse (RN) - Board of Nursing
- Active license in the state of practice
- Upon Hire: 1-3 years nursing
- Preferred: Certified Hemodialysis Nurse (CHN), Certified Peritoneal Dialysis Nurse (CPDN), Certified Dialysis Nurse (CDN), Specialty Certification - The American Society of Histocompatibility & Immunogenetics (ASHI)
- Upon Hire or Certified: Certified Hemodialysis Nurse (CHN), Certified Peritoneal Dialysis Nurse (CPDN), Certified Dialysis Nurse (CDN), Specialty Certification - The American Society of Histocompatibility & Immunogenetics (ASHI)
- Dialysis experience or critical care experience
Skills
- Visual acuity adequate to perform position duties
- Effective communication skills
- Ability to hear, understand and distinguish speech and other sounds
