Registered Nurse (RN) - Home Care Services

About the role

Confluence Health is seeking a clinically competent, registered professional nurse to coordinate and direct home health patient care. This role involves assessment, planning, intervention, implementation, and evaluation under physician or APP direction, with a focus on educating patients, families, caregivers, and community providers. The position requires ongoing evaluation of care plans and collaboration with healthcare teams to ensure high-quality, compassionate care.

Responsibilities

• Coordinate and provide high-quality patient care using the nursing process and unit-specific standards.
• Collect patient information to ensure safe, high-quality care and complete holistic ongoing assessments.
• Develop, implement, and continually reassess individualized patient care plans.
• Administer medications, perform nursing procedures/treatments, and address physical, psychological, social, and rehabilitative patient needs.
• Teach patients, families, and caregivers appropriate health information and share written materials in a timely manner.
• Evaluate and adjust discharge plans for appropriateness, educational needs, and changes as needed.
• Document care, assessments, interventions, and responses accurately in the patient’s medical record.
• Handle emergency situations promptly, precisely, and professionally.
• Maintain current knowledge of workflows, processes, and evidence-based procedures.
• Delegate tasks appropriately and oversee care provided by non-licensed assistive personnel.
• Participate in performance evaluation/improvement processes and assume responsibility for professional development.
• Communicate and collaborate with healthcare team members, including physicians, patients, and families.
• Maintain a clean and safe environment for patients and staff according to hospital and unit policies.
• Actively participate in quality improvement programs and adhere to Confluence Health policies and unit standards.
• Perform other duties as assigned and demonstrate standards of behavior and adherence to the Code of Conduct.

Requirements

• Current licensure in the state of Washington (RCW 18.88) or licensure through the Multistate Nurse Licensure Compact (SSB 5499).
• Basic Life Support (CPR) certification (must obtain within 90 days of hire).
• Current, valid driver’s license and access to an insured vehicle with minimum legal coverage.
• Basic computer skills related to Windows navigation, mouse usage, keyboarding, email communication, and password management.
