Responsibilities

  • Perform comprehensive patient assessments in the home setting
  • Develop and implement individualized care plans and goals
  • Provide skilled nursing care and treatments according to physician orders
  • Educate patients and families on disease management, medications, and care plans
  • Monitor patient progress and adjust care plans as needed
  • Document care accurately in the electronic medical record
  • Collaborate with physicians, therapists, and care team members to coordinate care
  • Maintain patient confidentiality and adhere to professional and regulatory standards

Requirements

  • Graduate of an accredited professional nursing program required
  • Bachelor of Science in Nursing (BSN) preferred
  • Minimum two (2) years of medical/surgical nursing experience preferred
  • Experience or knowledge in community health or home health nursing preferred
  • Current Registered Nurse (RN) license in Massachusetts required
  • BLS Basic Life Support (Healthcare Provider) certification required

Benefits

Compensation includes hourly pay for training, remote patient monitoring, education, orientation, meetings, and other non-clinical responsibilities. Clinical visits are paid at a base rate of $65 per visit, adjusted according to case weight.

Pay Range: $34.67 - $68.68 / Hourly

  • Shift: Day shift
  • Hours: 8:00am-4:30pm
  • Per Diem
