Registered Nurse, RN - Full Time, 8am - 4pm - AMG Hypertrophic Cardiomyopathy - Morristown

About the Role

As a Registered Nurse in Ambulatory care, you will use the nursing process to assess, plan, implement, and evaluate patient care. You will manage patient teaching, communication, triage, and documentation in Epic, ensuring patient safety, privacy, and timely response to clinical needs. You will collaborate with the care team, support quality improvement, promote evidence-based practice, and uphold PRIDE values and organizational goals.

Responsibilities

• Utilize the nursing process effectively (assessing, planning, implementing, and evaluating care) across the healthcare continuum.
• Demonstrate competent clinical skills and coordinate patient assessments to positively impact quality outcomes.
• Assess barriers to learning and functional status; coordinate, evaluate, and complete patient/family teaching.
• Respond to patients/families courteously, positively, and in a timely manner while respecting privacy and dignity.
• Uphold patient confidentiality according to HIPAA guidelines and foster a positive environment aligned with PRIDE values.
• Review and coordinate closure of care gaps to promote better patient outcomes.
• Interact directly with patients and team members, documenting in Epic as appropriate.
• Facilitate interdisciplinary collaboration and manage in-basket tasks.
• Assist with pharmacy contact, medication ordering, renewal, and lab dissemination based on acuity.
• Respond to patient messages in collaboration with clinicians and recognize emergency situations.
• Maintain a safe environment using HRO safety principles and report all injuries via the event reporting system.
• Demonstrate knowledge of medication/vaccine administration, monitor effects, and report adverse reactions.
• Follow policies/procedures, including emergency equipment use, and report risk management issues.
• Triage phone calls using critical thinking to evaluate urgency and assist in scheduling appointments.
• Provide comprehensive medical record documentation, maintaining patient confidentiality.
• Participate in practice huddles, communicate effectively with team members, and resolve conflicts.
• Mentor team members and address patient care concerns.
• Follow standard precautions, isolation techniques, and Ambulatory Health Care National Patient Safety Goals.
• Collaborate with the healthcare team to promote evidence-based care within a healing culture.
• Document patient responses to treatment, report condition changes, and verify order accuracy.
• Maintain competency in information systems and communicate effectively verbally and electronically.
• Pursue professional development through formal and independent learning activities.
• Monitor quality dashboards for assigned clinicians/practice.
• Perform other duties as needed within the scope of RN license.
• Support strategic goals of the clinical practice, AMG, AHS hospital sites, and the system.

Qualifications

• High School Diploma or GED.
• Graduate of an accredited School of Nursing.
• RN license valid in the State of New Jersey.
• American Heart Association-Basic Life Support (BLS) credential.
• Preferred: At least 2 years of clinical practice in ambulatory care.
• Preferred: Bachelor's degree in Nursing.
