About the role
Registered Nurse Case Managers provide comprehensive care that includes not just medical assistance, but also attentive listening and honoring patient and family wishes. Their primary role involves building trusting relationships with patients and their families, serving as the central point of communication within an interdisciplinary team of holistic care providers.
Responsibilities
- Build trusting relationships with patients and their families
- Provide holistic care that goes beyond medical assistance
- Honor patient and family wishes while ensuring dignity and comfort
- Navigate patients through one of life's most challenging journeys
Qualifications
- 6 months minimum prior skilled nursing experience
- An active unencumbered state Registered Nurse license
- A current driver's license and auto liability coverage
- An interest and ability to provide patient care in homes and facilities
Schedule & Benefits
- Schedule: Monday to Friday, 8:30 AM - 5:00 PM
- No mandatory on-call required
