Registered Nurse RN - Case Manager

Part-time, benefited position scheduled for 24 hours per week. Orientation lasts 6-8 weeks (Monday through Friday, 8 a.m. to 5 p.m.).

About the Role

The Registered Nurse Case Manager assumes overall accountability for patients’ discharge plans while leading efforts to achieve desired clinical, financial, and resource utilization outcomes. This role coordinates care and services for defined patient populations during an acute care episode across the continuum of care, supporting Trinity Health Livingston (Brighton).

Responsibilities

• Conduct focused assessments of patient goals, biophysical, psychosocial, environmental, economic/financial, and discharge planning needs.
• Review, on a concurrent basis, the appropriateness, intensity, and timeliness of the level of care, diagnostic testing, clinical procedures, quality, clinical risk issues, and documentation completeness.
• Develop and document a discharge plan in collaboration with the patient, family, and interdisciplinary team.
• Ensure all activities to facilitate and coordinate the plan are implemented and modify the plan as needed based on patients’ evolving needs.
• Discuss the plan of care daily with physicians and care team members to ensure the continued need for acute care hospitalization.
• Explore the most desirable and appropriate post-acute care setting to meet patients' needs and goals.

Requirements

• Current Michigan Nursing License.
• Graduation from an accredited nursing program; BSN is required.
• Three (3) years of recent experience in direct patient care as an RN with demonstrated clinical expertise or equivalent experience and expertise as a Case Manager.
• Case Management certification is desirable.
• Individuals serving as case managers must obtain CMSA certification within three years of employment in the role or be in the process of obtaining certification.
