Shift Description
The position is a Part-Time, benefited position, scheduled 24 hours per week. Orientation lasts 6-8 weeks from 8 a.m. to 5 p.m. Monday through Friday.
About the Role
The RN Registered Nurse Case Manager assumes overall accountability for patients’ discharge plans while leading in achieving clinical, financial, and resource utilization outcomes. This involves coordinating care and services for defined patient populations across the continuum of care.
Responsibilities
- Conducting a focused assessment of patient goals, biophysical, psychosocial, environmental, economic/financial, and discharge planning needs
- Reviewing and ensuring the appropriateness, intensity, and timeliness of the level of care, diagnostic testing, and clinical procedures
- Collaborating with the patient, family, and interdisciplinary team to develop and document a discharge plan
- Implementing the discharge plan and modifying it as needed based on patients' evolving needs
- Daily discussions with physicians and care team members to assess the need for acute care hospitalization and explore appropriate post-acute care settings
Requirements
- Current Michigan Nursing License
- Bachelor of Science in Nursing (BSN) required
- Three (3) years recent experience in direct patient care as an RN with demonstrated clinical expertise or equivalent experience and expertise as a Case Manager
- Certification in Case Management desirable
Qualifications
- Minimum qualifications include a current Michigan Nursing License and a BSN
- Recent experience as an RN with at least three years of relevant experience
- Desirable certification in Case Management
