Responsibilities
- Conduct in-home nursing visits for clients weekly, lasting 1-2 hours per visit
- Perform physical assessments including vitals
- Ensure home safety with medical equipment and supplies
- Fill weekly medication planners
- Oversight of skilled and non-skilled caregivers providing care in the client's home
- Plan, implement, and evaluate care provided
- Coordinate and manage client care conferences
- Support the development of qualifications and the assignment of personnel
- Participate in interviewing, selection, and ongoing evaluation of clinical personnel
- Train and educate clinical personnel
- Validate competency of clinical personnel
- Document and review nurse shift reports for adherence to policy and performance improvement
- Perform supervisory visits as indicated to facilitate problem resolution
- Ensure adherence to Agency policies
- Adhere to nursing delegation guidelines
- Ensure home chart completeness and timeliness of staffing cases post-referral
- Track equipment usage
- Participate in marketing activities
Requirements
- Graduate of a state-approved school of professional registered nursing
- BSN preferred
- Current, unrestricted RN license in the state(s) of practice
- Minimum of two (2) years nursing experience including one (1) year in home care or closely related field
- One (1) year of supervisory and/or case management experience preferred
- Current CPR certification
