About the role
The Hospice RN Case Manager and Home Health RN Case Manager roles are combined positions within Good Samaritan Hospice. Both roles are dedicated to providing high-quality, patient-centered care that promotes dignity, independence, comfort, and healing.
Responsibilities
- Provide case management for assigned hospice patients
- Conduct comprehensive assessments and ongoing visits
- Develop, implement, and update individualized Plans of Care with the Interdisciplinary Team (IDT)
- Manage pain and symptom control in collaboration with the physician
- Educate patients and families on medications, symptom management, and end-of-life care
- Career coordination and ongoing evaluation of patient and family needs
- Coordinate care with physicians, interdisciplinary team members, and caregivers
- Provide skilled nursing interventions including wound care, IV therapy, and symptom management
- Participate in IDT meetings, on-call rotation, and quality improvement initiatives
- Administer skilled nursing care to patients requiring intermittent professional services
- Complete admissions, re-certifications, recertifications, and discharge assessments
- Develop, implement, and revise Plans of Care based on patient needs
- Complete OASIS documentation accurately and within required timeframes
- Coordinate care with physicians, therapists, and interdisciplinary team members
- Provide patient and caregiver education on disease process, medications, and self-care
- Monitor patient progress and adjust care plans as needed
- Participate in case conferences, on-call rotation, and administrative meetings
Requirements
- Graduate of an accredited nursing program
- Minimum of 2 years nursing experience (home health or hospice experience, acute care, or geriatrics preferred; OASIS experience strongly preferred)
- Active Virginia RN license (or compact license)
- Current BLS/CPR certification or obtained within 30 days of hire
- Strong clinical assessment, communication, and decision-making skills
- Comfort with EMR systems and mobile documentation tools
- Valid driver’s license, reliable transportation, and proof of insurance

Schedule & Location
- Schedule: Monday–Friday, approximately 8:30 AM – 5:00 PM (with participation in on-call rotation as required)
