About the role

The Hospice RN Case Manager and Home Health RN Case Manager roles are combined positions within Good Samaritan Hospice. Both roles are dedicated to providing high-quality, patient-centered care that promotes dignity, independence, comfort, and healing.

Responsibilities

  • Provide case management for assigned hospice patients
  • Conduct comprehensive assessments and ongoing visits
  • Develop, implement, and update individualized Plans of Care with the Interdisciplinary Team (IDT)
  • Manage pain and symptom control in collaboration with the physician
  • Educate patients and families on medications, symptom management, and end-of-life care
  • Career coordination and ongoing evaluation of patient and family needs
  • Coordinate care with physicians, interdisciplinary team members, and caregivers
  • Provide skilled nursing interventions including wound care, IV therapy, and symptom management
  • Participate in IDT meetings, on-call rotation, and quality improvement initiatives
  • Administer skilled nursing care to patients requiring intermittent professional services
  • Complete admissions, re-certifications, recertifications, and discharge assessments
  • Develop, implement, and revise Plans of Care based on patient needs
  • Complete OASIS documentation accurately and within required timeframes
  • Coordinate care with physicians, therapists, and interdisciplinary team members
  • Provide patient and caregiver education on disease process, medications, and self-care
  • Monitor patient progress and adjust care plans as needed
  • Participate in case conferences, on-call rotation, and administrative meetings

Requirements

  • Graduate of an accredited nursing program
  • Minimum of 2 years nursing experience (home health or hospice experience, acute care, or geriatrics preferred; OASIS experience strongly preferred)
  • Active Virginia RN license (or compact license)
  • Current BLS/CPR certification or obtained within 30 days of hire
  • Strong clinical assessment, communication, and decision-making skills
  • Comfort with EMR systems and mobile documentation tools
  • Valid driver’s license, reliable transportation, and proof of insurance

Qualifications

Schedule & Location

  • Schedule: Monday–Friday, approximately 8:30 AM – 5:00 PM (with participation in on-call rotation as required)
  • Location: Roanoke and surrounding service areas
