Registered Nurse - RN Case Manager

Responsibilities

• Create and manage care plans for clients
• Conduct in-home nursing visits weekly, lasting 1-2 hours each
• Ensure home safety and medical equipment are properly managed
• Oversee and assist skilled and non-skilled caregivers
• Collaborate with the regional director and other personnel to address issues and improve services
• Coordinate and manage client care conferences
• Support the on-site clinical resource and ensure client needs are met
• Manage staffing and scheduling of personnel
• Adhere to nursing delegation guidelines and Agency policies
• Document and evaluate care provided
• Participate in performance improvement initiatives
• Interview, select, and evaluate clinical personnel
• Train and validate competency of clinical personnel
• Review and document supervisory visits
• Monitor home chart completeness and timeliness of staffing cases
• Assist with marketing activities

Qualifications

• Graduate of a state-approved school of professional registered nursing
• Bachelor of Science in Nursing (BSN) preferred
• Current, unrestricted RN license in the state(s) of practice
• Minimum of two (2) years nursing experience, including one (1) year in home care or closely related field
• One (1) year of supervisory and/or case management experience preferred
• Current CPR certification

Pay and Schedule

The standard rate is $44.00 hourly, plus shift differentials, where applicable.
