Responsibilities

  • Conduct in-home nursing visits for clients weekly, typically lasting 1-2 hours per visit.
  • Perform physical assessments including vitals, ensure home safety, fill weekly medication planners, and oversee caregivers.
  • Collaborate with the regional director and other personnel to identify and correct issues and improve services.
  • Plan, implement, and evaluate care provided.
  • Coordinate and manage client care conferences as needed.
  • Serve as a local on-site clinical resource and support the care of clients.
  • Assist and collaborate with staffing coordinators regarding staffing and scheduling of personnel.
  • Adhere to nursing delegation guidelines and ensure adherence to agency policies.
  • Document and report on infections, occurrences, complaints, and grievances.
  • Participate in interviewing, selection, and ongoing evaluation of clinical personnel.
  • Train and educate clinical personnel and validate their competencies.
  • Review and document supervisory visits to facilitate problem resolution.
  • Monitor and document home chart completeness, timeliness of staffing cases, equipment tracking, and perform marketing activities.
  • Perform additional duties as requested by the regional director.

Requirements

  • Graduate of a state-approved school of professional registered nursing.
  • Bachelor of Science in Nursing (BSN) preferred.
  • Current, unrestricted RN license in the state(s) of practice.
  • Minimum of two (2) years nursing experience, including one (1) year in home care or closely related field.
  • One (1) year of supervisory and/or case management experience preferred.
  • Current CPR certification.

Qualifications

Pay

$42.00 hourly plus shift differentials, where applicable.
