Registered Nurse - RN Case Manager

Responsibilities

• Conduct in-home nursing visits for clients
• Ensure home safety with medical equipment and supplies
• Fill weekly medication planners
• Oversight of skilled and non-skilled caregivers
• Plan, implement, and evaluate care provided
• Coordinate and manage client care conferences
• Support to ensure client’s home care needs are met
• Manage staffing and scheduling of personnel
• Adhere to nursing delegation guidelines
• Ensure adherence to Agency policies
• Document and submit data on infections, occurrences, complaints and grievances
• Perform supervisory visits
• Review nurse shift reports
• Participate in interviewing, selection, and ongoing evaluation of clinical personnel
• Train and educate clinical personnel
• Validate competency of clinical personnel
• Review and document clinical documentation
• Participate in performance improvement initiatives
• Assist with marketing activities

Qualifications

• Graduate of a state approved school of professional registered nursing
• BSN preferred
• Current, unrestricted RN license in the state(s) of practice
• Minimum of two (2) years nursing experience including one (1) year in home care or closely related field
• One (1) year of supervisory and/or case management experience preferred
• Current CPR certification

Standard Rate: $42.00 Hourly plus shift differentials, where applicable.
