About the Role
The Registered Nurse Case Manager assumes overall accountability for patients’ discharge plans while leading efforts to achieve desired clinical, financial, and resource utilization outcomes. This is accomplished by coordinating care and services for defined patient populations during an acute care episode across the continuum of care.
Orientation for this position lasts 6-8 weeks, Monday through Friday, 8 a.m. to 5 p.m.
Responsibilities
- Conduct a focused assessment of patient goals, biophysical, psychosocial, environmental, economic/financial, and discharge planning needs.
- Review, on a concurrent basis, the appropriateness, intensity, and timeliness of the level of care, diagnostic testing, clinical procedures, quality and clinical risk issues, and documentation completeness.
- Develop and document a discharge plan through collaboration with the patient, family, and interdisciplinary team.
- Ensure all activities to facilitate and coordinate the discharge plan are implemented and modify the plan as needed based on patients’ evolving needs.
- Discuss the plan of care daily with physicians and care team members to ensure the continued need for acute care hospitalization.
- Explore the most desirable and appropriate post-acute care setting to meet patients' needs and goals.
Requirements
- Current Michigan Nursing License.
- Graduation from an accredited nursing program; BSN is required.
- Three (3) years of recent experience in direct patient care as an RN with demonstrated clinical expertise or equivalent experience and expertise as a Case Manager.
- Case Management certification is desirable.
- Individuals serving as case managers must obtain CMSA certification within three years from their date of employment or be in the process of obtaining certification.
