Registered Nurse - RN Case Manager

Responsibilities

• Create and manage schedules independently
• Conduct in-home nursing visits for clients with chronic and terminal illnesses
• Ensure home safety and medical equipment maintenance
• Oversee and assist skilled and non-skilled caregivers
• Collaborate with regional directors and other personnel to address issues and improve services
• Plan, implement, and evaluate care provided
• Attend and manage client care conferences
• Support and assist in staffing coordination
• Adhere to nursing delegation guidelines and agency policies
• Document and report on infection rates, occurrences, complaints, and grievances
• Participate in performance improvement initiatives
• Train and educate clinical personnel
• Facilitate problem resolution through supervisory visits
• Review and validate competency of clinical staff
• Manage equipment tracking and home chart completeness
• Engage in marketing activities as needed

Qualifications

• Graduate of a state-approved school of professional registered nursing
• Bachelor of Science in Nursing (BSN) preferred
• Current, unrestricted RN license in the state(s) of practice
• Minimum of two (2) years nursing experience, including one (1) year in home care or closely related field
• One (1) year of supervisory and/or case management experience preferred
• Current CPR certification

Pay

$44.00 per hour plus shift differentials, where applicable.
