Responsibilities
- Create and manage schedules independently
- Conduct in-home nursing visits for clients with chronic and terminal illnesses
- Ensure home safety and medical equipment maintenance
- Oversee and assist skilled and non-skilled caregivers
- Collaborate with regional directors and other personnel to address issues and improve services
- Plan, implement, and evaluate care provided
- Attend and manage client care conferences
- Support and assist in staffing coordination
- Adhere to nursing delegation guidelines and agency policies
- Document and report on infection rates, occurrences, complaints, and grievances
- Participate in performance improvement initiatives
- Train and educate clinical personnel
- Facilitate problem resolution through supervisory visits
- Review and validate competency of clinical staff
- Manage equipment tracking and home chart completeness
- Engage in marketing activities as needed
Qualifications
- Graduate of a state-approved school of professional registered nursing
- Bachelor of Science in Nursing (BSN) preferred
- Current, unrestricted RN license in the state(s) of practice
- Minimum of two (2) years nursing experience, including one (1) year in home care or closely related field
- One (1) year of supervisory and/or case management experience preferred
- Current CPR certification

Pay
$44.00 per hour plus shift differentials, where applicable.
