Registered Nurse - RN Case Manager

Responsibilities

• Conduct in-home nursing visits for clients weekly, typically lasting 1-2 hours per visit
• Do physical assessments including vitals, ensure home safety with medical equipment and supplies, fill weekly medication planners, and oversee caregivers
• Plan, implement, and evaluate care provided
• Coordinate and manage client care conferences
• Support staffing coordinators regarding staffing and scheduling
• Ensure adherence to Agency policies and perform supervisory visits
• Document and report on infections, occurrences, complaints, and grievances
• Participate in performance improvement initiatives
• Assist with marketing activities

Requirements

• Graduate of a state-approved school of professional registered nursing
• BSN preferred
• Current, unrestricted RN license in the state(s) of practice
• Minimum of two (2) years nursing experience including one (1) year in home care or closely related field
• One (1) year of supervisory and/or case management experience preferred
• Current CPR certification
