Responsibilities

  • Create and manage care plans for assigned clients.
  • Conduct in-home nursing visits weekly, lasting 1-2 hours each.
  • Ensure home safety and medical equipment are properly managed.
  • Oversee and coordinate care provided by skilled and non-skilled caregivers.
  • Collaborate with the regional director and other personnel to address issues and improve services.
  • Manage client care conferences as needed.
  • Support the on-site clinical resource and ensure client needs are met.
  • Coordinate staffing and scheduling of personnel.
  • Adhere to nursing delegation guidelines and Agency policies.
  • Document and evaluate care provided, ensuring accuracy and completeness.
  • Participate in performance improvement initiatives and assist with marketing activities.
  • Perform supervisory visits and review nurse shift reports.
  • Assist with data collection and submission for infection control, complaints, and grievances.
  • Assist with marketing activities such as client and physician visits.

Qualifications

  • Graduate of a state-approved school of professional registered nursing.
  • Bachelor of Science in Nursing (BSN) preferred.
  • Current, unrestricted RN license in the state(s) of practice.
  • Minimum of two (2) years nursing experience, including one (1) year in home care or closely related field.
  • One (1) year of supervisory and/or case management experience preferred.
  • Current CPR certification.

Pay

Senior Case Manager Availability

The senior case manager, or similarly qualified alternate, shall be available at all times during operating hours and participate in all activities relevant to the professional services furnished.
