About the role

The Registered Nurse Case Manager (RNCM) assumes ultimate accountability and leadership for the assessment of the patient condition and plan of care. The RNCM provides case management, clinical care and is responsible for coordination all disciplines involved in providing quality, cost-effective and billable care through direct care and supervision of care under the direction of the physician.

Responsibilities

  • Assesses the patient condition and develops a plan of care.
  • Provides case management, clinical care, and coordinates care among various healthcare professionals.
  • Educates patients and their families about their health conditions and treatment plans.
  • Maintains patient safety and ensures compliance with all legal rules and regulations.
  • Completes accurate documentation and maintains compliance with state nursing standards.
  • Works collaboratively with other healthcare providers and staff to ensure seamless patient care.

Requirements

  • Minimum of one year of experience as an RN in an acute care setting.
  • Home care experience is desired.
  • Effective communication and the ability to create positive impressions with patients, families, physicians, and co-workers.
  • Calmness, patience, and accommodating nature.
  • Compassion and caring attitude.
  • Knowledgeable on nursing best practices.
  • Ability to make appropriate nursing judgments.
  • Ability to identify situations and provide the best possible solutions.
  • Detail-oriented and observant.
  • Dedicated work ethic with the ability to prioritize and be timely.
  • Able to follow directions and work as a team member.

Qualifications

  • Registered Nurse with current license in the state of employment.
