Registered Nurse (RN) Case Manager
About the role
The Registered Nurse (RN) Case Manager is responsible for managing a defined caseload of patients providing direct patient care and comprehensive nursing services to individuals in the community.
Responsibilities
- Manage a defined caseload of patients providing direct patient care and comprehensive nursing services to individuals in the community.
- Ensure compliance with all regulatory standards and policies.
- Collaborate with interdisciplinary teams to develop and implement care plans.
- Assess patient needs and provide appropriate interventions.
- Document patient care activities accurately and timely.
- Communicate effectively with patients, families, and other healthcare providers.
- Participate in ongoing education and training programs.
Requirements
- Valid nursing license in the state of NH
- 2+ years of experience in skilled care preferred
- 1+ year of experience in nursing supervision preferred
- Recent wound care experience preferred
- Current CPR certification
- Reliable transportation
