Schedule: Monday-Friday | 8:30 AM - 5:00 PM

What You'll Do
- Conduct comprehensive patient assessments and develop individualized plans of care.
- Provide skilled nursing visits and case management services.
- Career coordination with physicians, therapists, caregivers, and other healthcare professionals.
- Maintain patient progress and adjust care plans as needed.
- Provide patient and family education to promote independence and improved health outcomes.
- Complete timely and accurate clinical documentation.
- Supervise and collaborate with interdisciplinary team members to ensure exceptional patient care.
- Advocate for patients while promoting quality, safety, and continuity of care.

Qualifications
- Current Registered Nurse (RN) license in the State of New Hampshire.
- Minimum of two (2) years of skilled nursing experience preferred.
- Previous nursing supervisory experience preferred.
- Recent wound care experience preferred.
- Current CPR Certification.
- Reliable transportation and valid driver's license.
- Strong assessment, communication, organizational, and clinical decision-making skills.
