Registered Nurse (RN) Care Navigator
Purpose & Scope
Supports patients across the continuum of care by coordinating transitions of care, conducting patient outreach, supporting quality and care gap closure, and facilitating effective care coordination within the ambulatory setting between patients, providers, and post‑acute settings.
Functions
Functions within a multidisciplinary care team to improve patient experience, clinical outcomes, and performance on quality and population health initiatives.
Education
- Required: Associates Degree in Nursing
- Prior to 2024: Preferred: Bachelor’s Degree in Nursing
Experience
- Required: 3 years of healthcare, or nursing/patient care experience
- Prior experience as a Nurse Navigator in transitions of care, care coordination, outreach, ambulatory care, population health, and/or quality program support preferred
Certification/Licensure
- Licensed as a Registered Nurse by the Virginia Department of Health Professions or compact state licensure is required.

The compensation offered for this position—whether expressed as an annual salary or hourly rate—is determined based on a variety of legitimate, non-discriminatory factors. Schedule
No portion of compensation is considered earned until it is vested and determined in accordance with the terms of applicable policies and benefit plans. The availability and amount of any bonuses, incentives, or other benefits are subject to the terms of those plans and may be modified at the sole discretion of VHC Health, in accordance with applicable law.
