Registered Nurse (RN) Care Coordinator PRN

Role: The Care Coordinator RN (CC RN) assesses transitional care needs, plans and coordinates patient care across the continuum, and engages with patients and families to ensure care goals are met. Responsibilities: Conduct psychosocial and functional assessments, develop individualized discharge plans, facilitate interdisciplinary communication, and manage barriers to timely discharge. Collaborate with physicians, care teams, and social services to promote efficient care progression and resource utilization. Document assessments, care plans, and interactions accurately, track avoidable days, and support post-acute referrals. Ensure compliance with policies and participate in professional development activities. Qualifications & Skills: Requires an associate's or bachelor's nursing degree, valid RN license (single-state or multi-state compact), BLS certification, and at least one year of acute care nursing experience. Must have excellent communication, assessment, organizational, and problem-solving skills, with knowledge of healthcare regulations and community resources. Ability to work collaboratively, handle stress, and adapt in a fast-paced environment is essential.
