Responsibilities
- Collaborate with clinic team members to provide evidence-based, patient-centered care, helping patients stay up to date on their care plans and related health goals.
- Focus on high-risk, medically complex patients, those on specific disease registries, patients with disorders or chronic physical/intellectual disabilities, and patients in care transitions (e.g., pediatric to adult care, recent inpatient admissions/discharges, ED follow-up, care facility discharges).
- Act as a liaison with patients' family members and other agencies to facilitate improved communication and provide comprehensive whole-person care.
- Stay up to date on standards and assist with programs and processes.
Clinic operating hours: Monday–Friday, 8:00 AM–9:30 PM; Saturday and Sunday, 8:00 AM–8:00 PM.
Qualifications
- Associate Degree in Nursing from a CCNE (Commission on Collegiate Nursing Education) or ACEN (Accreditation Commission for Education in Nursing) accredited nursing program.
- Current Ohio Registered Nurse license.
- CPR/BLS certified.
- Knowledge of triage procedures and experience in clinic, physician office, and/or urgent care settings.
- Ability to maintain quality, safety, and infection control standards.
- Knowledge of procedures and standards for administering medications and patient care aids.
- Ability to educate and clearly communicate with patients and families about disease nature, care, and treatment.
- Strong written and verbal communication skills.
- High level of interpersonal skills to handle sensitive and confidential situations with poise, tact, and diplomacy.
- Analytical ability and critical thinking to gather and interpret data, and develop, recommend, and implement solutions.

Pay
USD $32.21 - USD $47.60 per hour.
