Registered Nurse (RN) - Care Coordination FT Days

About the role

This position is for an RN Care Coordinator responsible for assessing patients' transitional care needs, coordinating care across the continuum, and engaging with patients and families to ensure seamless care progression and discharge planning.

Responsibilities

• Conduct psychosocial and clinical assessments of patients.
• Facilitate interdisciplinary rounds to coordinate care.
• Manage discharge plans and address barriers to timely discharge.
• Collaborate with physicians, care teams, and social services to develop individualized care and discharge plans.
• Ensure effective resource utilization and patient safety.
• Maintain accurate documentation of care coordination activities.
• Pursue professional development opportunities.
• Support team goals to enhance patient care and organizational success.

Requirements

• Minimum of 1 year of acute care nursing experience.
• Active RN license (single or multi-state).
• Basic Life Support (BLS) certification.
