Registered Nurse Care Coordinator FT Days

The Care Coordinator RN (CC RN) is responsible for assessing patients' transitional care needs, coordinating care across the continuum, and collaborating with patients, families, physicians, and interdisciplinary teams to ensure effective discharge planning and care progression. Key Duties Include Conducting psychosocial and functional assessments, developing individualized discharge plans, and facilitating patient/family education.Collaborating with care teams and utilizing resources to address barriers, expedite care, and ensure timely discharges.Managing all aspects of discharge planning, including post-acute referrals and documentation in medical records.Identifying delays, resolving obstacles, and advocating for patient needs throughout the care process.Participating in interdisciplinary rounds and maintaining accurate, timely documentation. Qualifications & Skills Minimum of 1 year of acute care nursing experience.Excellent communication, organizational, and problem-solving skills.Knowledge of healthcare regulations and community resources.Licensure as a Registered Nurse (RN) with BLS certification.Ability to work collaboratively in a fast-paced environment and handle complex cases.
