Registered Nurse Care Coordinator FT Days

The Care Coordinator RN (CC RN) plays a vital role in assessing patients' transitional care needs, coordinating across the continuum, and engaging with patients and families to ensure comprehensive care and timely discharge. Responsibilities Include Conducting psychosocial and functional assessments, developing discharge plans, and facilitating patient/family education.Collaborating with physicians, care teams, utilization review nurses, and social workers to evaluate medical, psychosocial, and social determinants of health needs.Identifying and resolving barriers to discharge, managing post-acute referrals, and ensuring resource availability.Documenting assessments, care plans, and interactions accurately and promptly.Participating in interdisciplinary rounds and ensuring seamless communication for efficient patient care progression.Supporting professional development, mentoring, and adhering to organizational policies.
