At Brighton/Suncrest Hospice our goal is to change the expectation of hospice care in your area by providing exceptional care and service to our patients. This is achieved by allocating the resources to increase our staff‑to‑patient ratios, thereby increasing clinical visits while lowering clinician caseloads. Responsibilities

• Provide care and services for a group of patients, visiting patients where they reside (private homes, Skilled Nursing Facilities, and assisted living facilities).
• Serve as a contributing member of the interdisciplinary team, offering expertise in psychosocial assessments, interventions, and knowledge of community resources.
• Work in one of three focus areas: Case Management, Admissions, or Resource role.
• Collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership.
• Engage with patients on a one‑to‑one level while receiving strong support from the patient‑care team.
• Manage independent day‑to‑day activities including phone, email, written communication, meetings with healthcare professionals, and adherence to hospice rules and regulations.
• Prioritize and triage multiple concerns, schedule visits, attend required meetings, and respond to urgent needs.

Requirements

• Current Registered Nurse (RN) licensure in good standing in the state(s) of practice.
• Ability to work effectively within a team environment and collaborate with interdisciplinary staff.
• Capacity to work independently, managing phone, email, written communication, and meetings.
• Understanding of hospice rules and regulations.
• Strong time‑management skills to manage visits, documentation, meetings, and urgent priorities.
• Willingness and ability to learn an electronic medical records (EMR) system.
• Reliable transportation, a valid driver’s license, and auto insurance to travel across the service area.
