Responsibilities

The Registered Nurse (RN) will be responsible for providing care and services for a group of patients, visiting patients where they reside, which could include private homes, Skilled Nursing Facilities, and assisted living facilities. This position is a contributing member of the interdisciplinary team, providing expertise regarding psych-social assessments, interventions, and knowledge of community sources. The Registered Nurse (RN) will work in either a Case Management, Admissions, or Resource role. Emphasis placed on Spanish-speaking candidates.

Requirements

• Current Registered Nurse (RN) licensure (in good standing) in the state(s) of practice
• Willing to drive with reliable transportation, valid driver’s license, and auto insurance to travel across the service area

Skills

• Works well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership
• Ability to work closely with patients on a 1‑to‑1 level while receiving strong support from a patient care team
• Capable of working independently on a day‑to‑day basis, handling phone, email, written communication, meetings with various healthcare professionals, and understanding hospice rules and regulations
• Strong time‑management skills to manage visits, documentation, meetings, prioritize and triage urgent needs
• Ability to learn an electronic medical records system
