This is achieved by allocating the resources to increase our staff to patient ratios, thereby increasing clinical visits while lowering clinician caseloads. Responsibilities

• Provide care and services for a group of patients, visiting patients where they reside (private homes, skilled nursing facilities, assisted living facilities).
• Contribute as a member of the interdisciplinary team, providing expertise regarding psychosocial assessments, interventions, and knowledge of community sources.
• Work in either a Case Management, Admissions, or Resource role.

Requirements

• Current Registered Nurse (RN) licensure (in good standing) in the state(s) of practice.

Skills

• Work well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership.
• Work closely with patients on a 1‑to‑1 basis while receiving strong support from the patient care team.
• Ability to work independently day‑to‑day, handling phone, email, written communication, meetings with various healthcare professionals, and understanding hospice rules and regulations.
• Strong time‑management skills to manage visits, documentation, and meetings (scheduled visits, required meetings, urgent needs) by prioritizing and triaging multiple concerns.
• Ability to learn an electronic medical records system.
• Willingness to drive with reliable transportation, a valid driver’s license, and auto insurance to travel across the service area.

Pay

• Pay range: $78,000‑$90,000 per year.
• Monthly bonus opportunities.
