Registered Nurse (RN)

About the Role

The registered nurse plans, organizes and directs hospice care and is experienced in nursing, with emphasis on community health education and experience. The professional nurse builds from community resources to plan and direct services that meet the needs of individuals and families within their homes and communities.

Responsibilities

• Complete an initial, comprehensive assessment of patient and family to determine hospice needs, including a full physical assessment and illness history.
• Provide professional nursing care by utilizing all elements of the nursing process as defined in the state Nurse Practice Act.
• Assess and evaluate the patient’s status, write and initiate a plan of care, regularly re‑evaluate patient and family/caregiver needs, and revise the plan as necessary.
• Use health assessment data to determine nursing diagnoses and develop a care plan with goals, incorporating palliative nursing actions and involving the patient and family in planning.
• Initiate appropriate preventive and rehabilitative nursing procedures; administer medications and treatments as prescribed by the physician.
• Counsel the patient and family, provide health‑care instructions, and assist with activities of daily living to promote self‑sufficiency and comfort.
• Act as Case Manager when assigned, coordinating patient care for an assigned caseload.
• Complete, maintain, and submit accurate clinical notes, recording pain and symptom management changes and outcomes.
• Communicate with physicians regarding patient needs, obtain orders, and relay changes in condition.
• Coordinate with community health personnel to align the care plan.
• Teach patients and caregivers self‑care techniques, medication, diet, and other instructions; provide health counseling as appropriate.
• Work collaboratively with the interdisciplinary team, maintaining a safe environment for the patient.
• Assist the patient, family, and team in providing continuity of care and meeting emotional needs.
• Attend interdisciplinary group meetings and participate in on‑call duties as defined by policy.
• Ensure necessary equipment and services are available for patient care.
• Supervise ancillary personnel, delegating responsibilities when required.
• Pursue personal growth and professional development through continuing education and in‑service classes.
• Fulfill case assignments and actively participate in quality assessment and performance‑improvement activities.

Qualifications

• Graduate of an accredited school of nursing.
• Current licensure as a Registered Nurse in the state of Montana.
• One to two years of recent acute‑care experience in an institutional setting.
• One to two years of recent home‑care experience.
• Bachelor’s degree (preferred) with at least one year of hospice care experience preferred.
• Must be a licensed driver with an insured automobile that meets state and organization requirements and is in good working order.
• Excellent observation, verbal and written communication, problem‑solving, and basic math skills; nursing skills per competency checklist.
• Physical ability to walk or stand for prolonged periods, lift, position, and transfer patients, lift supplies and equipment, and perform reaching, stooping, bending, kneeling, or crouching as needed.
• Adequate visual acuity and hearing to perform required nursing skills.
• Management experience not required; responsible for supervising hospice aides.

Pay

$75,000 – $82,000 per year
