Registered Nurse (RN)

About the role

The Registered Nurse Care Manager represents the interests of Veteran patients throughout the clinic and coordinates care with the VA Medical Center and stakeholders. This role requires an understanding of the clinic’s clinical and medical programs, processes, and procedures, and is directly linked to customer service and the Service Excellence program initiatives. Under the direction of the Community Based Outpatient Clinic (CBOC), the RN provides professional clinical services and manages patient care. Hiring for this position is contingent upon STGi’s selection as the successful awardee of an active Request for Proposals (RFP).

Responsibilities

• Provide nursing care based on physician, mid‑level provider, and CBOC Clinic Manager orders to meet patients’ physical, mental, and emotional needs.
• Supervise support staff, if applicable, in the delivery of care.
• Lead and participate in PACT team huddles.
• Work with the Primary Care Physician to ensure adequate staffing for quality care and promote continuity of care between the VAMC and the clinic.
• Maintain universal precautions and infection control practices.
• Supervise medical assistants and/or licensed practical nurses who obtain initial patient information (vital signs, symptoms, complaints) through interviewing and record review.
• Prepare patients for examination and assist the PACT Provider upon request.
• Ensure smooth and timely patient flow in accordance with VA access standards and triage protocol.
• Demonstrate, understand, and perform triage activities for walk‑in patients and medical phone inquiries.
• Administer medications per facility protocols as prescribed by the provider.
• Perform CPR and assist during respiratory and cardiac arrest procedures.
• Perform a range of nursing procedures including dressing changes, suture removal, oxygen administration, wound irrigations, catheterizations, and venipunctures.
• Prepare patients for laboratory procedures by providing information and instructions.
• Set up and operate therapeutic and emergency equipment such as monitors, ECG, oxygen, and portable suction.
• Document all pertinent patient information in medical records and VISTA to demonstrate quality care delivery and promote continuity of care.
• Keep the VISTA clinic reminder tracking system current for each enrolled patient.
• Assist in scheduling patients for initial and follow‑up appointments per VA access standards and medical necessity.
• Keep patients informed of their plan of care, including lab and diagnostic test results as needed.
• Provide health materials, resources, and education to patients and families according to need and learning capacity.
• Assist providers in ensuring all patient information on electronic encounter documentation is complete and accurate.
• Participate in the ongoing Performance Improvement Program between subcontractor(s) and the VAMC.
• Ensure all required reports, clinical and administrative documentation are completed accurately, timely, and completely.
• Participate in the management of day‑to‑day CBOC operations.
• Comply with all federal, state, local, Joint Commission, OSHA, VA, subcontractor safety and operational regulations, directives, and standards.
• Maintain confidentiality of all information and support patients’ privacy, rights, and safety.
• Perform the duties of the CBOC Clinic Manager when he/she is absent (if applicable).
• Perform other work‑related duties as assigned.

Requirements

• Minimum of 3 years of experience in a Medical Surgical Unit, Outpatient Facility, Primary Care, and/or Adult Health setting.
• Current and unrestricted nursing license in the state of practice.
• Current CPR/BLS certification from AHA or MTN.
• RN graduate of a National League for Nursing (NLN)‑accredited school.
• Bachelor of Science in Nursing (BSN) preferred.
• Ability to perform moderate physical activity, handling objects up to 25 pounds and standing/walking more than four hours per day.
• Ability to respond to medical emergencies within the clinic requiring a fast walk or jog.

• Ability to assess patient condition, recognize adverse signs and symptoms, and react swiftly in emergency situations.
• Ability to set realistic, measurable education goals for patients, considering patient/family needs, level of understanding, and available local resources.
• Ability to develop and provide individualized and group counseling, guidance, and health instruction focused on disease prevention and health maintenance.
• Ability to teach patients and/or families proper home health care activities (e.g., giving injections, taking blood pressures, changing wound dressings).
• Knowledge of pharmacological agents used in patient treatment, including desired effects, side effects, complications, and usage considerations.
• Knowledge of basic customer service techniques related to age‑specific populations.
• Effective verbal and written communication skills with proper telephone etiquette.
• Proficiency with Microsoft Office software, Electronic Medical Records, and general computer maneuverability.
• Exceptional diplomatic communication skills and experience handling difficult customer situations.
• Detail‑oriented with strong multitasking ability.
