Registered Nurse (RN)
Job Description
Establishes a trusted partnership with the member and builds creditability through knowledge sharing and education around healthcare needs while using critical thinking and prioritization in collaboration with the care team.
Provides a high degree of customer service including receiving member incoming calls (return calls and/or triage), in-person interactions, and member outreach as needed.
Administers and documents member intake, point of care testing, immunizations, injections, oral medications, assists providers with procedures and performs phlebotomy (as needed).
Affords care coordination including pre-visit planning, clinical pathway adherence, care gap identification and management, ER/inpatient discharges and transitions of care in conjunction with the care team.
Operates as one of the liaisons for member healthcare needs and utilizes all forms of communication effectively to maximize a collaborative relationship in identifying member needs for Behavioral Health, Health Coaching, Diabetic Educators and BCBS programs.
Documents and completes tasks assigned to inboxes within the EHR system and member call queues.
Works with the care team to meet quality performance targets and metrics established for the providers and care center.
Maintains confidentiality of all regulated information in compliance with state and federal laws; reads, understands and follows medical policy and all corporate, divisional and departmental policies and procedures, including but not limited to: Protected Health Information (PHI) and Personal Identifiable Identification (PII).
May be required to travel between Care Centers.
Minimum Qualifications
- Associate or Bachelor’s degree in Nursing
- Valid and active Registered Nurse (RN) in Kansas and Missouri or multi-state compact license
- BLS certification from American Red Cross or American Heart Association
- Maintain licensures and certifications throughout employment
- Intermediate knowledge/experience with common diagnoses typically seen in primary care
- Ability to work effectively in a fast-paced environment with competing priorities
- Ability to define problems (technical and clinical), collect data, establish facts and draw valid conclusions and action plans
- Intermediate knowledge of Microsoft Windows applications
- Ability to work with a variety of member tools/applications used to access and document health information and support member interactions through various communication channels (phone, in-person, chat, email, etc.)

Preferred Qualifications
- Bachelor’s degree
- Prior primary care experience
- Athena EHR experience
- Experience with ACA and/or Medicare populations
- Advanced knowledge/experience with common diagnoses typically seen in primary care
