Key Duties
- Developing, implementing, and updating personalized care plans in collaboration with healthcare teams
- Supervising home health aides and licensed nurses, providing instruction, and ensuring compliance with standards
- Conducting comprehensive assessments, accurate documentation, and medication reconciliation
- Ensuring quality care through effective communication with patients, families, and healthcare providers
- Participating in team activities, orientation, quality improvement, and survey readiness
- Adapting to diverse home environments and promoting patient safety and positive outcomes
Qualifications
- Current, unrestricted RN license in the practicing state
- Valid driver's license, reliable transportation, and insurance
Preferred Skills And Experience
- CPR certification (or ability to obtain within 90 days)
- At least 1 year of home health experience
- Strong communication, organizational, and independent working skills
