Registered Nurse (RN)

About the Role

Mountainside Nursing and Rehab, a quality skilled nursing facility in Mountainside, New Jersey, offers short and long-term nursing and rehabilitation care. This position provides basic medical attention to patients, assesses their needs, and delivers direct nursing care to assigned residents.

Responsibilities

• Provide direct nursing care to assigned residents in accordance with the nursing process.
• Direct day-to-day functions of Licensed Practical Nurses and Certified Nursing Assistants, ensuring compliance with state and federal regulations and facility policies.
• Assess and respond to resident conditions utilizing the nursing process.
• Maintain effective communication with the interdisciplinary team to ensure the highest standard of care for each resident.
• Complete and ensure LPN and C.N.A. documentation of care and services rendered according to facility policies.
• Complete reports such as the 24-hour change in condition, Incident and Accident reports, etc., as assigned by the Director of Nursing Services.
• Ensure timely and accurate medication and treatment administration.
• Communicate to physicians and document changes in resident condition using both objective and subjective nursing assessment.
• Document and implement changes in nursing and medical plans of care.
• Ensure all members of the interdisciplinary team are aware of and understand changes in resident nursing and/or medical plans of care, per facility policy.
