Assists the care team for provision of participant care including after hours/weekend on-call rotation.\ \ Participate in the development, implementation, and evaluation of the Interdisciplinary Team (IDT) plan of care and update changes as necessary.\ \ Attend Interdisciplinary Team (IDT) care plan meetings and Participant/Family meetings as applicable. May perform assessments in a variety of settings including PACE Clinic and/or Adult Day Center.\ \ Develop and provide education to participants, caregivers, and associates as necessary including end of life discussions.
